Medical Records Go Where You Need Them

A detailed record of your medical history is kept in our office so your doctor can refer to this important information when seeing you. These records are the property of the Medical Center and for your protection, are considered confidential.

If you would like copies of your medical records or x-rays sent to a physician outside the Medical Center, we are happy to comply with your requests. Medical records and x-rays are released only with the signed Record Release Authorization form by the patient, legal guardian, or an individual with power of attorney. Forms are available at the reception desk or on this website. Please allow 7 - 10 business days for your records to be copied. There may be a fee charged for the copies of your records.
