New patients are an important part of our practice. Our physicians welcome new patients by self-referral or referrals from other physicians outside the Medical Center.

If you are a new patient and would like to schedule an appointment, we can assist you in selecting a Primary Care Physician for you and your family — or a specialist, if you have a specific health problem.

Appointment availability depends on the nature and urgency of the medical problem. Appointments for urgent health needs are scheduled promptly, often for the same day.

Prior to your first appointment, we will need to complete a Patient Registration Summary. We may do this over the phone or in person when you arrive for your appointment. You may print this form from this website and bring the completed form with you. This information is used to establish a patient record with information for contacting you regarding your appointment, sending test results, or contacting a family member in case of an emergency. We also use this information to submit the fees for the services you received to your health insurance company.
