Please call our Administration Department (620-669-6620) to set up an industrial account for your company prior to scheduling any industrial medicine services. Please see the Industrial Account Profile Form on this website for the information needed to establish an industrial medicine account.

Workers' Compensation Services

The Medical Center Central office and the Medical Center South Hutch office provide workers' compensation services from 8:00 a.m. to 4:30 p.m., Monday through Friday. Injuries outside of office hours are referred to the Hospital ER.

Initial Contact

Please call our Administration Department to set-up an industrial account for your company prior to scheduling any workers' compensation services. Please see the Industrial Account Profile Form on this website for the information needed to establish an industrial medicine account.

When a worker is injured, please contact the Medical Center to advise us a worker has been injured and is being sent to our office. At the time of the initial contact, we will need the patient’s name, date of birth, Social Security Number, and the type and severity of the injury. Additional information can be completed by the patient/employee after arrival.

The injured worker will be seen by the physician as soon as possible upon arrival. In the event the injury requires immediate attention, blood control, etc. the worker will be immediately directed to our Ancillary Department, which is staffed with nursing personnel trained to provide immediate First Aid and attend the patient until the physician is available.

Request for Special Services

The Medical Center can provide many services in addition to the evaluation and treatment of injuries. Some of these services include post-accident drug testing and Breath Alcohol testing (BAT). Please advise the receptionist at the initial contact if any of these services are to be performed. Special services are noted on our Industrial Account Profile Form, which is kept on file for instructions to our staff of the services requested.

Limited Duty and Return to Work

Upon evaluation and treatment of the injury, a Return to Work or Restriction form will be completed by the physician and given to the employee to forward to his/her employer.

It is the goal of the Medical Center to assist employers with identifying light duty tasks that can be temporarily assigned to a worker until they are ready to be returned to their previous work duties, full time. It is helpful if a physical task description or job description is available to the physician. An on-site tour of the working environment is also very helpful. If at any time there are any questions about availability of light duty options, the physician may contact the employer for more information.

A copy of the Form G and/or progress notes dictated by the treating physician will be sent to the employer or insurance company, giving the results of the examination, and the diagnosis and treatment of the patient.

Subsequent Services

Follow-up visits are scheduled as needed.

Billing

Services provided for on-the-job injuries are billed according to the current State of Kansas Workers' Compensation Fee Schedule. Itemized claim forms are forwarded to the employer along with the medical documentation for those services, unless we are instructed to forward them directly to your Workers' Compensation Insurance Carrier. Any questions regarding the billing can be directed to our Insurance Supervisor.
